Requirements
- Current, unrestricted RN license in the state of practice.
- Minimum of one year of nursing experience in an acute care, skilled nursing, rehabilitation, long-term care, or other clinical setting.
- Ability and willingness to travel throughout the assigned service area.
- Valid driver's license, reliable transportation, and current auto insurance in accordance with state requirements.
- Ability to lift, push, pull, and support up to 50 pounds, including assisting patients and moving equipment when necessary.
- Associate Degree in Nursing required
Nice To Haves
- Bachelor's Degree in Nursing preferred.
- Previous Home Health or Hospice experience is preferred but not required.
- Experience with HomeCare HomeBase (HCHB) is preferred.
Responsibilities
- Provide skilled nursing care to patients in their homes throughout your assigned territory.
- Complete comprehensive assessments to identify patient, caregiver, and environmental needs.
- Develop, implement, and manage individualized plans of care designed to improve patient outcomes and promote independence.
- Serve as the primary case manager for your assigned patient caseload.
- Perform admissions, routine visits, re-certifications, resumptions of care, and discharge visits as appropriate.
- Monitor patient progress and adjust care plans based on changing clinical needs.
- Collaborate with physicians, therapists, social workers, aides, and other interdisciplinary team members to ensure coordinated care.
- Educate patients and caregivers on disease management, medications, treatments, and safety strategies.
- Communicate significant changes in patient condition to providers and clinical leadership promptly.
- Complete timely and accurate clinical documentation in accordance with company, state, and federal requirements.
- Maintain patient confidentiality and privacy in accordance with HIPAA regulations.
