Requirements

  • Current, unrestricted RN license in the state of practice.
  • Minimum of one year of nursing experience in an acute care, skilled nursing, rehabilitation, long-term care, or other clinical setting.
  • Ability and willingness to travel throughout the assigned service area.
  • Valid driver's license, reliable transportation, and current auto insurance in accordance with state requirements.
  • Ability to lift, push, pull, and support up to 50 pounds, including assisting patients and moving equipment when necessary.
  • Associate Degree in Nursing required

Nice To Haves

  • Bachelor's Degree in Nursing preferred.
  • Previous Home Health or Hospice experience is preferred but not required.
  • Experience with HomeCare HomeBase (HCHB) is preferred.

Responsibilities

  • Provide skilled nursing care to patients in their homes throughout your assigned territory.
  • Complete comprehensive assessments to identify patient, caregiver, and environmental needs.
  • Develop, implement, and manage individualized plans of care designed to improve patient outcomes and promote independence.
  • Serve as the primary case manager for your assigned patient caseload.
  • Perform admissions, routine visits, re-certifications, resumptions of care, and discharge visits as appropriate.
  • Monitor patient progress and adjust care plans based on changing clinical needs.
  • Collaborate with physicians, therapists, social workers, aides, and other interdisciplinary team members to ensure coordinated care.
  • Educate patients and caregivers on disease management, medications, treatments, and safety strategies.
  • Communicate significant changes in patient condition to providers and clinical leadership promptly.
  • Complete timely and accurate clinical documentation in accordance with company, state, and federal requirements.
  • Maintain patient confidentiality and privacy in accordance with HIPAA regulations.
