About The Position

This is a "Road Warrior" leadership position with our agency, requiring travel as needed to all clients throughout Northwest Ohio. This role works closely with and reports directly to our NW OH Regional Program Administrator. For those pursuing career growth: a defined path for advancement to Assistant Program Administrator, Program Administrator, Clinical Administrator, Director of Nursing (DON or Asst. DON), Business Administrator, QA / Compliance leadership, and other positions will be supported. Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall. To support our growth and the increasing volume of client care throughout Ohio, we are seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager (RNCM) / Recertification Nurse to join our agency and serve existing and returning clients throughout Northwest Ohio in a full-time capacity. Requirements

  • At least 1 year of experience in Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
  • At least 1 year of paid experience at/with a home health agency or in-home care provider (or equivalent).
  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • Minimum of 1 year of experience in home health agency or equivalent setting.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • Experience with OASIS and NON-OASIS assessments is also required.
  • Strong background in nursing, patient care, and confidentiality.
  • Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid).
  • Willingness and ability to perform required physical demands.
  • Consent + Ability to pass a State & Federal background check (including fingerprints).
  • 18yo + active Driver's License / State ID + US Work Authorization.
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius and assigned clients (up to at least 2 hours is required).

Responsibilities

  • Case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services.
  • New hire orientations, in-service and supervisory reviews.
  • Provide input to influence training and education, care practices and outcomes, and the success of clients, employees and business overall.
  • Back-up and occasional Skilled Nursing visits and services including med sets, dressing changes, etc.
  • Coordinating, re-evaluating and confirming the medical necessity of continued home health care services for our patients.
  • Providing patient care and ensuring optimal outcomes.
  • Overseeing patient services and providing expert nursing care.
  • Submitting updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines.
  • Ensuring the successful communication with and between physicians, agency staff, service administrators, other caregivers and clinicians, and the client and his/her family.
  • Performing client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • Managing/supervising a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • Documenting/Communicating/Coordinating patient status/change in condition to care team.
  • Performing job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
