Home Health Registered Nurse
About The Position
As a CenterWell Home Health nurse, the Registered Nurse reports to the Clinical Manager and provides personalized, one-on-one care to help patients regain independence in their homes. Working with physicians and other clinicians, the nurse manages care plans that support patient recovery. The position provides skilled nursing care in patients’ homes, including medication administration, wound care, and patient assessments. The nurse monitors patients’ conditions, documents care, coordinates with healthcare professionals, educates patients and families, reports progress to physicians and the Clinical Manager, and oversees Home Health Aides and their documentation.
Requirements
- Med-surg, ICU, ER, or acute experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver’s license, auto insurance, and reliable transportation.
- One year of nursing experience.
- Home Health experience.
Responsibilities
- Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments.
- Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
- Monitor patients’ conditions and report changes.
- Educate patients and their families on disease management, medication, and treatment options.
- Maintain accurate records of patient care and coordinate with other healthcare professionals.
- Report patient care and condition progress to the patient’s physician and Clinical Manager.
- Oversee Home Health Aides and monitor their appropriate completion of documentation.
