About The Position

Well Care Health is seeking a passionate Home Health Registered Nurse to join their growing team and provide exceptional care in a supportive and rewarding environment. This is a full-time position where RN Case Managers deliver one-on-one care to patients inside the comfort of their own homes. The role involves creating and maintaining the patient plan of care in coordination with the physician, providing direct care to 5-6 patients per day, and coordinating with the assigned LPN. Patient interventions include wound care, infusion care, med/surg care, medication teaching, and cardiac teaching. Clinicians must be comfortable using computers and smartphones, as the agency is 100% EMR. Reliable transportation and exceptional time management skills are also required.

Requirements

Graduate of an accredited or approved school of nursing, either an AD, Diploma, or BSN program.
Current license to practice professional nursing in the State of North Carolina.
Requires significant standing and walking.
Frequent lifting greater than 50 pounds and transferring patients greater than 100 pounds.
Requires repeated reaching in front of the body, bending and stopping.
Must have current NC Drivers License with clean driving record.
Must have an operational vehicle.
Comfortable using computers and smartphones.
Exceptional time management skills.

Responsibilities

Assess and monitor patients' health conditions in their home environment.
Develop and implement individualized care plans.
Educate patients and families on health management and self-care.
Collaborate with a multidisciplinary team to ensure optimal patient outcomes.
Create and maintain the patient plan of care in coordination with the physician.
Provide direct care to 5-6 patients per day.
Coordinate with the assigned LPN.
Perform patient interventions including wound care, infusion care, med/surg care, medication teaching, and cardiac teaching.
