About The Position

Elara Caring is hiring a full-time Home Health RN Case Manager for the Arlington/Belmont area. The role provides skilled nursing care in patients’ homes and supports recovery, health management, and independence. The nurse guides patients through home health care from assessment through discharge, while managing an assigned caseload and coordinating care with physicians and other clinicians. The position offers a guaranteed salary of $90,000–$115,000 based on experience, plus a productivity bonus.

Requirements

Current, unrestricted Massachusetts RN license.
At least one year of clinical nursing experience.
Strong assessment, communication, and time management skills.
Ability to work independently and travel throughout the assigned territory.
Valid driver’s license, current auto insurance, and reliable transportation.
Ability to sit, stand, bend, lift, and move intermittently, and lift 50–100 pounds.

Responsibilities

Provide skilled nursing visits in patients’ homes.
Guide patients through home health care from assessment to discharge.
Develop and update patient care plans.
Manage an assigned caseload.
Coordinate care with physicians and other clinicians.
Teach patients and caregivers to manage medications, treatments, and health conditions at home.
