This role involves empowering patients and families to reach healthcare goals through education on disease management, and case managing and providing clinical oversight to an interdisciplinary team. Requirements
- RN license in the state you work
- Two years of experience as a registered nurse
- Current driver’s license
- Current CPR and BCLS certification
