HOME HEALTH REGISTERED NURSE
About The Position
Skilled nursing services shall be provided by a registered nurse in accordance with the plan of treatment with regards to the applicable nurse practice act. Nursing care shall include, but not be limited to, the promotion, maintenance, and restoration of health, and ensuring the prevention of infection, accident, and injury. The registered nurse will perform initial and ongoing periodic assessments, initiate preventative and rehabilitative nursing procedures, and notify physicians of significant changes in patient condition. The role involves reassessing patient needs, monitoring response to care, coordinating services, preparing documentation, and teaching, supervising, and counseling patients, families, and staff. The registered nurse may delegate tasks to competent non-licensed personnel working under supervision. Knowledge of KINNSER documentation and OASIS assessment completion is required.

Responsibilities
- Provide nursing services in accordance with the plan of treatment.
- Promote, maintain, and restore health, and prevent infection, accident, and injury.
- Perform initial nursing assessment prior to care provision, and provide ongoing periodic assessment.
- Initiate preventative and rehabilitative nursing procedures.
- Notify the patient's attending physician, dentist, or podiatrist and other professional persons and responsible staff of significant changes in the patient's condition.
- Reassess the patient's nursing care needs on an ongoing, patient-specific basis and provide care consistent with the medical plan of treatment.
- Monitor the patient's response to nursing care.
- Assist in coordinating all services provided.
- Prepare documentation and clinical notes.
- Teach, supervise, and counsel the patient, family members, and staff regarding nursing care and the patient's needs.
- Delegate tasks to non-licensed personnel, ensuring they are competent and work under supervision.
- Complete OASIS assessments.
