About The Position

Sage Group HealthCare Staffing is seeking a Home Health RN for a 13-week travel/contract position in Lacey, WA. This role involves providing and coordinating care and case management for assigned home health patients in their homes. The nurse will develop and implement nursing care plans, perform case management, admissions, and various types of visits. A key aspect of the role is to support patient comfort, pain management, and the ability to remain at home, empowering patients and families through education on disease and pain management. The position requires adherence to state Nurse Practice Act regulations and facility policies, ensuring physician orders are transcribed correctly, and patients' status is continuously assessed and documented. The nurse will also notify physicians of significant changes in patient status and complete all charting within 24 hours. Bereavement support for patient families is also part of the role. The territory to cover is Lewis County with potential crossover, requiring flexibility in coverage areas.

Requirements

  • Registered Nurse: 1 year - Required
  • Recent/Current RN Home Health experience: 1 year within the last 3 years – Required
  • Home Care Home Base (HCHB) charting - Required
  • OASIS experience - Required
  • Must have valid Driver’s License and Personal Auto Insurance
  • ASN or an equivalent – Required
  • RN License for State of the state applying or Compact if acceptable - Required
  • BLS Certification from AHA or American Red Cross - Required

Nice To Haves

  • BSN or an equivalent – Highly preferred

Responsibilities

  • Provide and coordinate care and case management for assigned home health patients in comfort of their homes.
  • Develop and implement nursing care plans including SOC, and all types of visits as assigned.
  • Performs Case Management, Admissions and all types of visits for the assigned patient case load
  • Support patient comfort, pain management and ability to stay at home - enabling them to spend quality time with their loved ones.
  • Empower patients and families by educating them on disease management and pain management.
  • Provide Home health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
  • Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
  • Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
  • Notify physicians of significant changes in status.
  • Document and complete charting for all patient visits within 24 hours.
  • Provide bereavement support to patient’s families as needed.
