About The Position
Concierge Home Care is seeking a Home Health Registered Nurse (RN) to provide skilled nursing care in patients' homes, supporting their recovery and independence. This role offers flexibility, supportive leadership, and modern technology, allowing nurses to focus on patients and their quality of life. The position is located in Jacksonville, FL, with a coverage area including Arlington, Downtown, and Southside.
Requirements
- Active, unencumbered Florida RN license
- Valid driver’s license, reliable transportation, and auto insurance
Nice To Haves
- Home health experience preferred but not required
Responsibilities
- Conduct patient assessments and develop care plans
- Provide skilled nursing care and patient education
- Coordinate care with physicians and interdisciplinary teams
- Monitor patient progress and adjust care as needed
- Document visits efficiently using AI-assisted tools
