We specialize in home health care services that enable the patient to adapt and manage his or her medical situation in the comfort of their home. Stella Maris offers home health services to residents of Baltimore City, Baltimore County, and Harford County. Stella Maris is looking for Home Health Registered Nurses (RN) to join their growing Home Health team. This is an exciting opportunity to work alongside dedicated clinicians to meet the needs of clients in the community we serve. Requirements
- RN licensure in the State of Maryland required
- Prior experience in hospice or home health required
- Valid driver’s license and reliable transportation
- Commitment to Stella Maris philosophy of care, values, mission statement and organizational culture
Nice To Haves
- OASIS Documentation experience preferred
Responsibilities
- Performs primary nursing care functions in conformance with professional standards of home health nursing practice.
- Provides individualized care to patients in their home under the plan of care approved by the primary physician.
- Coordinates all services provided for each assigned patient and family.
- Provides leadership and advocacy in the coordination of patient care with multiple clinicians, modalities, and community resources.
- Manages clinical resources and transition planning for patients within the assigned caseload from pre-admission through post-discharge, actively working to provide excellent medical care.
- Requires daily travel in assigned region.
