Requirements

Active RN license
Active BLS certification
Active CPR certification
Experience with OASIS-based home health documentation
Experience with care planning, medication management, and coordination of care
Experience working with patients and families, physicians, therapists, and the full care team
Comfort with technology and electronic documentation systems
Eligible to work in the United States
Reliable transportation

Nice To Haves

Home health experience is a plus, but not required — we’re happy to train the right nurse
Bachelor of Science in Nursing (BSN) is a plus
Experience with Kinnser is a plus

Responsibilities

Provide one‑on‑one nursing care in the comfort of patients’ homes
Complete assessments, develop care plans, and deliver high‑quality clinical care
Educate patients and families to support independence and confidence
Collaborate with physicians, therapists, and your care team to ensure seamless care
Document visits accurately and timely using our user‑friendly EMR
Support patients through recovery, chronic condition management, and health maintenance
