About The Position

Elara Caring is hiring a full-time Home Health RN Case Manager for the Arlington/Belmont area. The nurse provides skilled care in patients’ homes, supporting recovery, health management, and independence. The role includes assessing patients, providing nursing visits, developing and updating care plans, managing an assigned caseload, and coordinating care with physicians and other clinicians. The nurse also teaches patients and caregivers about medications, treatments, and managing health conditions at home. The position offers a guaranteed salary of $90,000–$115,000 based on experience, plus a productivity bonus. Workday planning is flexible, and patient visits are one-on-one and clustered.

Requirements

• Current, unrestricted Massachusetts RN license.
• At least one year of clinical nursing experience.
• Strong assessment, communication, and time management skills.
• Ability to work independently and travel throughout the assigned territory.
• Valid driver’s license, current auto insurance, and reliable transportation.
• Ability to sit, stand, bend, lift, and move intermittently, and lift 50–100 pounds.

Responsibilities

• Provide skilled nursing visits in patients’ homes.
• Assess patients and guide them through home health care from assessment to discharge.
• Develop and update patient care plans.
• Manage an assigned caseload.
• Coordinate care with physicians and other clinicians.
• Teach patients and caregivers to manage medications, treatments, and health conditions at home.
