About The Position
Pathfinder Home Health is seeking compassionate PRN Registered Nurses in the Athens, Texas area. The role provides skilled nursing care in patients’ homes and supports individualized care plans in collaboration with an interdisciplinary team. Nurses assess patient progress, administer treatments, and communicate with patients, families, physicians, case managers, and the care team. The position uses AI-driven, voice-enabled, HIPAA-compliant documentation through the Roger app to reduce charting time and help clinicians meet OASIS requirements. Electronic charting is also provided through the Kinnser (WellSky) system. The organization emphasizes Honor, Kindness, and Patience, and offers clinical and administrative support around the clock.
Requirements
- Active RN license in Texas or a compact state.
- Active CPR certificate.
- Prior experience in geriatric home health care.
- Self-starter with organizational skills.
- Compassionate approach to care.
- Valid driver’s license.
Responsibilities
- Provide comprehensive skilled nursing care in patients’ homes in accordance with physician orders and individualized plans of care.
- Collaborate with the interdisciplinary care team to develop, implement, and update personalized care plans.
- Administer nursing treatments and manage clinical activities to support patient health outcomes.
- Assess patient progress and evaluate the effectiveness of services.
- Communicate assessment findings with the care team, patient, family, physician, and case manager.
- Perform additional duties as needed.
