About The Position
Outstanding Registered Nurses needed for field-based positions servicing multiple Santa Clara County locations: San Jose, Cupertino, Campbell, Saratoga, Morgan Hill and Gilroy. ACE Home Health and Hospice is a locally owned and operated agency with offices based in Orinda and San Jose, CA. We are growing and are seeking talented and compassionate professionals that want to make a difference in the lives of their patients. Positions can start as per diem and could quickly grow into full time employment. Requirements
- 1 year full time experience in Med/Surg or acute setting
- 1 year minimum home health experience
- RN unrestricted license in California
- CPR
- California Driver’s License
- valid auto insurance
- Skilled in IV infusion , phlebotomy, wound care including wound vacs, NGT/GT, teaching and management of all disease processes
- Able to manage an independent schedule
- supervise LVN's & CHHA's
- excellent communicate skills
- integrity and loyalty required
- Maintain flexibility with the challenges of Bay Area car travel from home to home
- Experienced in home health software and other electronic communication
- Critical thinking skills, time management and attendance at case conference/staff meetings a must
Nice To Haves
- Able to work one weekend per month is a plus
Responsibilities
- Responsible for the assessment and plan of care of home health patients in accordance with the RN scope of practice and in compliance with CMS guidelines and regulations for home health care.
- Responsible for creating the OASIS/start of care documentation, re-visits following start of care, care coordination and managing a team of therapists to ensure patients meet their home health goals, and can be discharge back to the community with emphasis on an excellent patient experience.
