Home Health Registered Nurse
About The Position
As a CenterWell Home Health nurse, reporting to the Clinical Manager, you provide personalized, one-on-one care to help patients regain independence at home. You work with physicians and other clinicians to manage care plans that support recovery. The role involves providing skilled nursing care in patients’ homes, including medication administration, wound care, and patient assessments. You develop and implement individualized care plans with healthcare professionals, monitor patient conditions, educate patients and families, maintain accurate records, coordinate care, and report patient progress to physicians and the Clinical Manager. You also oversee Home Health Aides and monitor their documentation.
Requirements
- Med-surg, ICU, ER, or acute experience.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Valid driver’s license, auto insurance, and reliable transportation.

- One year of nursing experience.

Responsibilities
- Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments.

- Oversee Home Health Aides and monitor appropriate completion of their documentation.
