This is an opportunity to provide meaningful, one-on-one care to patients in the comfort of their own homes while working with a supportive and collaborative clinical team. The Registered Nurse (RN) will deliver high-quality home health care within a defined local territory (approximately a 30-minute radius). This role focuses on patient-centered care, clinical excellence, and independence in the field.

Requirements

• Associate Degree in Nursing (or higher)
• Active Registered Nurse (RN) license in the State of Michigan
• Minimum of 1 year of RN experience
• AHA BLS (Basic Life Support) certification
• AHA ACLS (Advanced Cardiac Life Support) certification
• Valid driver’s license, reliable transportation, and auto insurance
• Willingness to travel within assigned territory

Nice To Haves

• Home Health experience strongly preferred
• Clean nursing license
• Experience with adult patient populations (non-pediatric focused)

Responsibilities

• Provide skilled nursing care to patients in their homes
• Perform assessments, develop care plans, and monitor patient progress
• Educate patients and families on care plans and health management
• Collaborate with interdisciplinary teams to ensure optimal outcomes
• Maintain accurate and timely documentation
