About The Position
Columbia River Home Health is expanding its team and looking for a compassionate Home Health Registered Nurse Case Manager (RN) in the Tri-Cities area. They offer a ride-along program for those new to home health. Requirements
- Current RN license in Washington State
- Minimum 2 years nursing experience
- CPR certification
- Licensed driver with reliable, insured vehicle
- Strong communication, clinical judgment, and patient-focused care skills
Nice To Haves
- Home health experience preferred but not required
Responsibilities
- Complete home health intake assessments, medication reconciliation, and care plans
- Collaborate with physicians, caregivers, and interdisciplinary teams
- Educate patients, families, and caregivers on disease management, safety, and recovery
- Maintain accurate clinical documentation and communication with staff
