About The Position

Interim HealthCare is hiring PRN Hospice Registered Nurses (RN) to join their team in Rochester, MN and surrounding areas. This role focuses on providing comfort, strength, and guidance to patients and families during life's final chapter. Requirements

• Graduate of an accredited nursing program and active RN license in Wisconsin.
• Minimum of 1 year of nursing experience, ideally in hospice or home healthcare.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Clinical proficiency in nursing specialty, strong communication skills and compassion.

Responsibilities

• Provide home-based care to patients with six months or less to live, focused on quality of life.
• Work as part of an interdisciplinary team to carry out the patient’s plan of care.
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments.
• Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies.
• Educate family on the patient’s prognosis, changes in care plan, medication and hospice goals.
• Take a holistic approach to care that engages the patient’s mind, body, spirit and family.
