Our staff volunteers with Meals on Wheels, we host monthly Veteran-to-Veteran coffees through our hospice program, and we show up for this city the same way we show up for our patients.

Requirements

• Active RN license in Oklahoma
• At least one year of nursing experience; home health experience preferred
• CPR certification
• Strong clinical judgment, communication skills, and a genuine passion for patient-centered care
• Reliable transportation

Responsibilities

• Provide skilled nursing care in the home following discharge from a hospital, rehab, or nursing facility
• Conduct assessments, document progress, and communicate changes to the physician
• Oversee LPNs, CNAs, and aides supporting your patients
• Educate patients and families on care plans, medications, and home safety
• Coordinate care goals and manage discharge from services
