About The Position
At Columbia River Home Health, we put people and culture first! We are expanding our team and looking for a compassionate Home Health Registered Nurse Case Manager (RN) in the Walla Walla area. Never worked in home health? No problem! We offer a ride-along program so you can see what a career in home health is really like.
Requirements
- Current RN license in Washington State
- Minimum 2 years nursing experience; home health experience preferred but not required
- CPR certification
- Licensed driver with reliable, insured vehicle
- Strong communication, clinical judgment, and patient-focused care skills
Nice To Haves
- Home health experience preferred but not required
Responsibilities
- Complete home health intake assessments, medication reconciliation, and care plans
- Collaborate with physicians, caregivers, and interdisciplinary teams
- Educate patients, families, and caregivers on disease management, safety, and recovery
- Maintain accurate clinical documentation and communication with staff
