About The Position
NurseAmerica Healthcare is seeking compassionate and experienced Home Health Registered Nurses (RNs) to provide high-quality, patient-centered care in a home setting. This role focuses on delivering skilled nursing services to patients with chronic, acute, and end-of-life care needs. You’ll play a critical role in improving patient outcomes while allowing individuals to receive care in the comfort of their own homes.
Requirements
- Active RN license (NH or compact state)
- Minimum 1 year of RN experience (Home Health or Hospice preferred)
- Strong clinical assessment and critical thinking skills
- Valid driver’s license and reliable transportation
- BLS certification required
Responsibilities
- Perform comprehensive patient assessments and develop individualized care plans
- Administer medications, treatments, and wound care
- Monitor patient progress and adjust care plans as needed
- Educate patients and families on disease management and care plans
- Collaborate with interdisciplinary teams (physicians, therapists, caregivers)
- Maintain accurate and timely documentation
