About The Position

We’re seeking a compassionate Registered Nurse (RN) to join our team in a full-time Home Health role. This is a case management role for home health patients where you will serve adult and geriatric patients in their homes or senior living communities. This is a longstanding, stable company with years of successful operation.

Requirements

• RN license in the state of CT
• Some experience in Home Health required
• US Citizen or Green Card Holder
• Comfortable with traveling between patient homes (expect 20–40 minutes of driving at times)
• Experience providing home health care to sub-acute medical/surgical patients with Medicare payer source, including full proficiency in completing OASIS documentation

Responsibilities

• Case management role for home health patients
• Direct patient scheduling = you control your start and end times
• Serve adult and geriatric patients in their homes or senior living communities
