About The Position

Valley Regional Home Health is currently seeking a full-time Registered Nurse for the Clovis, Fresno, and Madera areas. This position would be Monday through Friday. Training hours are generally 8:00am-4:30pm, but once the employee is independent, RNs can adjust their start times and end times to provide patient care in the home environment. Our home health RNs see between 3-5 patients per day and drive between patient visits. We are a locally owned and operated home health agency looking to add Nurses to its current long-term and experienced Nursing team. Requirements

  • Registered nurse with current licensure to practice professional nursing in the state.
  • Graduate of National League for Nursing accredited school of nursing.
  • Maintains a current CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order - as well as willing to drive around the job posting area.
  • Demonstrates excellent observation, problem solving, verbal and written communication skills; ability to work well with others and communicate time-sensitive information effectively.
  • Shows ability to organize and prioritize workload independently – home health employees are typically independent in the field.

Nice To Haves

  • One year of home health experience preferred, but not required.

Responsibilities

  • Completes an initial assessment of patient and family to determine home care needs.
  • Provides a complete physical assessment and history of current and previous illness(es).
  • Ability to read documentation on patient status as well as any orders that the patient PCP may have provided.
  • Regularly re-evaluates patient nursing needs and completes necessary charting within the required timeframe.
  • Initiates the plan of care and makes necessary revisions as patient status and needs change; uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures.
  • Administers medications, treatments, IV ABX, etc. as prescribed by the physician.
  • Counsels the patient and family in meeting nursing and related needs, provides education on plan of care.
  • Provides health care instructions to the patient as appropriate per assessment and plan of care.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
