About The Position

BAYADA Home Health Care is offering a unique opportunity for a Registered Nurse, RN with a passion for clinical education. In this role as a Full Time Registered Nurse Preceptor, you will train, coach and mentor newly hired RNS and LPNs. This exciting new role will work one-on-one with our nurses to ensure our clinicians are delivering exceptional care to adult and geriatric patients living in their homes throughout the greater Denver area. Previous Medicare Certified Home Health and OASIS documentation experience is required. This position is not a dedicated clinical educator role. The RN Preceptor maintains a caseload and patient care responsibilities. When onboarding needs arise, the RN Preceptor will also provide training, mentorship, and field-based support to new clinical hires. As a result, this role requires flexibility and the ability to travel throughout the office's entire coverage area to support onboarding activities. You will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to remain at home. Requirements

  • Current nursing license in good standing in the state of Colorado.
  • Graduation from an accredited and approved nursing program.
  • At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings).
  • Previous Medicare Certified Home Health and OASIS documentation experience is required.
  • Ability to work independently while knowing when to escalate concerns.
  • Comfortable making real-time clinical decisions without immediate on-site support.
  • Strong problem-solving skills in unpredictable home environments.
  • Ability to manage a dynamic visit schedule and prioritize urgent visits.
  • Efficient with point-of-care documentation and technology.
  • Strong attention to detail while maintaining productivity expectations.
  • Comfortable providing care in varied home environments.
  • Flexible and adaptable to changing patient needs and schedules.
  • Demonstrates cultural sensitivity and respect for patient autonomy.

Responsibilities

  • Train, coach and mentor newly hired RNs and LPNs.
  • Work one-on-one with nurses to ensure exceptional care to adult and geriatric patients.
  • Maintain a caseload and patient care responsibilities.
  • Provide training, mentorship, and field-based support to new clinical hires.
  • Use strong head-to-toe assessment skills with the ability to identify subtle changes in patient condition.
  • Utilize experience with acute and post-acute care (e.g., CHF, COPD, wound care, IV therapy, post-surgical care).
  • Triage and manage urgent clinical situations independently in the home setting.
  • Perform medication reconciliation, teaching, and safety monitoring.
  • Perform comprehensive wound assessment, care, and documentation.
