Requirements

Active Registered Nurse (RN) license
Current CPR Certification
Valid driver's license
Reliable transportation with current auto insurance

Nice To Haves

Minimum of 2 years of Home Health nursing experience
Homecare Homebase (HCHB) experience

Responsibilities

Complete comprehensive patient assessments, including physical, functional, psychosocial, and environmental evaluations.
Develop, implement, and update individualized plans of care to promote positive patient outcomes.
Determine patient eligibility for home health services and complete admission and recertification assessments.
Educate patients and caregivers on disease management, medications, nutrition, safety, and self-care.
Coordinate care with physicians and interdisciplinary team members.
Supervise and provide clinical direction to Home Health Aides and LPN/LVNs.
Perform skilled nursing interventions, treatments, and patient education.
Monitor patient progress and communicate changes in condition promptly.
Maintain accurate, timely documentation in accordance with regulatory and agency standards.
Ensure compliance with infection prevention practices and payer requirements.
Advocate for patients while helping them maintain independence and improve quality of life.
