About The Position

The After-Hours RN provides clinical support outside of traditional business hours and helps ensure continuity of care while reducing unnecessary hospitalizations. This role involves completing skilled nursing visits during evenings and weekends, responding to after-hours patient calls, coordinating care with physicians and interdisciplinary teams, and providing skilled clinical care including comprehensive patient assessments, medication reconciliation, wound care, disease management education, and catheter care. The position also supports urology-related patient needs and requires participation in rotating evening/weekend on-call coverage. Documentation must be completed within required EMR timeframes.

Requirements

• Active Florida RN license or Compact RN license
• Strong assessment and triage skills
• Ability to work independently
• Reliable transportation, valid driver's license, and auto insurance

Responsibilities

• Complete skilled nursing visits during evenings and weekends
• Perform Start of Care (SOC) assessments
• Perform Resumption of Care visits
• Perform PRN visits
• Perform Urgent nursing visits beginning at 4:00 PM
• Respond to after-hours patient calls and triage concerns appropriately
• Coordinate care with Physicians
• Coordinate care with Case managers
• Coordinate care with Interdisciplinary care teams
• Provide skilled clinical care including comprehensive patient assessments
• Provide skilled clinical care including medication reconciliation
• Provide skilled clinical care including wound care
• Provide skilled clinical care including disease management education
• Provide skilled clinical care including catheter care
• Support urology-related patient needs including Foley catheter management
• Support urology-related patient needs including Nephrostomy care
• Support urology-related patient needs including Urinary retention monitoring
• Support urology-related patient needs including Post-surgical urology follow-up
• Support urology-related patient needs including Infection prevention education
• Complete documentation within required EMR timeframes
• Participate in rotating evening/weekend on-call coverage
