About The Position

Compassionate Home Health LTD is seeking a Home Health Registered Nurse (RN) for per diem shifts in the Greater Sacramento Area. This role involves planning, coordinating, and documenting care for an assigned caseload of patients. The RN will develop and implement home health plans based on physician's orders and professional skills, ensuring compliance with accrediting standards, federal and state regulations, reimbursement guidelines, and agency policies. The position also includes participation in Quality Assessment and Performance Improvement (QAPI) activities.

Requirements

Licensed Registered Nurse in California
Credentials in specialty areas, i.e., phlebotomy, IV, ET.
Current BCLS, CPR card.
One (1) year of acute care nursing or Skilled Nursing experience.
Valid California Driver’s License, current automobile insurance, and reliable automobile.
Able to communicate effectively in English, both verbally and in writing.
Computer Knowledge to work on Electronic Medical Records (EMR)
Knowledgeable of current Federal, State, and Agency regulations on patient home healthcare.

Nice To Haves

Prior experience in home health is preferred, but we can train if a new grad (NEW GRAD IS WELCOME TO APPLY)
Additional languages preferred.

Responsibilities

Making the initial evaluation visit to Oasis Start of Care (SOC)
Regularly re-evaluating the patient’s nursing needs
Initiating the plan of care and necessary revisions
Furnishing those services requiring substantial and specialized nursing skill
Initiating appropriate preventive and rehabilitative nursing procedures
Preparing clinical and progress notes, coordinating services, and informing the physician and other personnel of changes in the patient’s condition and needs
Counseling the patient and family in meeting nursing and related needs
Participating in in-service programs, supervising, and teaching other nursing personnel
