About The Position

AdvisaCare is seeking skilled Registered Nurses (RN) or Licensed Practical Nurses (LPN) to provide high-quality home health care to patients in the Greater Muskegon and surrounding areas. This is a fast-paced role within a Medicare Division that is expanding. The ideal candidates will be dynamic, energetic, organized, and efficient professionals committed to providing customized care based on individual patient needs and preferences. The role involves a variety of nursing tasks and skills, including wound care (Wound Vacs, Drains, Tubes, Ostomies, Traches, Catheters, Plurex drains), infusions, and LVADs. The position emphasizes a holistic, patient-driven approach with one-on-one patient and caregiver education and coordination with the Home Health Team, offering a rewarding alternative to facility-based care. Training will be provided for advanced skills as needed, and OASIS training is available for exceptional candidates.

Requirements

• Current Michigan Registered Nurse (RN) or Licensed Practical Nurse (LPN).
• Minimum one (1) year of Home Health nursing experience preferred.
• Demonstrated knowledge and skills to provide care to and communicate with all ages of the population.
• Updated Tuberculin (TB) test.
• Current CPR certification.
• Clear Criminal History/Drug Screen.
• Compassionate and caring demeanor, with a professional presentation at all times.
• Ability to travel within service area of AdvisaCare (approximately a 30 mile radius from your home).
• Critical thinking, task competence and the ability to think on your feet is needed.

• OASIS Experienced Registered Nurse preferred.

Responsibilities

• Provide high-quality home health care to patients.
• Perform various nursing tasks and skills, including wound care (Wound Vacs, Drains, Tubes, Ostomies, Traches, Catheters, Plurex drains), infusions, and LVADs.
• Conduct one-on-one education with patients and their caregivers.
• Coordinate with all members of the Home Health Team.
• Travel within the service area of AdvisaCare (approximately a 30-mile radius from home).
