About The Position

This is a Full Time Home Health Float Pool position based in Wisconsin, supporting the northern patient service markets. The areas of work will be based on assignment. The typical schedule is Monday through Friday from 8:00 am to 4:30 pm, with some variability based on departmental needs. An every 4th weekend rotation is required.

Requirements

  • Associate’s degree in nursing.
  • Active RN license in state of practice.
  • Basic Life Support (BLS) – American Heart Association
  • Valid driver's license and reliable transportation.
  • 3-4 years clinical experience.
  • 1-2 years in home health.
  • Ability to work independently and make sound clinical decisions
  • Excellent communication (written and verbal) and interpersonal skills.
  • Ability to develop rapport and maintain positive, professional relationships with a variety of patients, staff and physicians.
  • Cultural competence and respect for patient diversity.
  • Good organization, prioritization and problem-solving skills.

Nice To Haves

  • Bachelor of Science in Nursing (BSN).

Responsibilities

  • Complete scheduled patient visits and collaborate with the interdisciplinary team.
  • Conduct comprehensive patient assessments and update plans of care as needed.
  • Communicate treatment changes with physicians and the primary registered nurse.
  • Monitor patient response to care and revise interventions accordingly.
  • Coordinate scheduling and case conferences with primary nurse when appropriate.
  • Assist with the discharge planning process according to agency standards and utilize interagency/system and community resources to ensure continuity of care after discharge.
  • Communicate problems to physicians and primary Registered Nurse, and clearly state plans, actions and goals so that changes are reflected as they occur.
  • Maintain accurate documentation in compliance with all regulations.
  • Follow agency policies and safely perform required patient care activities.
  • Provide age-appropriate care based on patient needs and developmental stage.
