As our Home Health Registered Nurse Clinical Manager, you’ll be the heartbeat of our clinical operations. Your days will be filled with purpose as you support and mentor a dedicated team of nurses and caregivers, ensure our patients receive safe, high-quality, personalized care, and oversee clinical documentation, care plans, and regulatory compliance. You will collaborate with physicians, referral partners, and families, lead case conferences and guide clinical decision-making, help onboard, train, and inspire new team members, and keep a close eye on quality measures, patient satisfaction, and outcomes. This RN role blends leadership, clinical expertise, and relationship-building — perfect for someone who loves both people and process.
Requirements
- Active MI RN license
- Solid understanding of Medicare home health
- Comfort with EMR systems and clinical workflows
- Experience working with physicians, referral partners, patient families, and interdisciplinary care teams
- Eligible to work in the United States
Nice To Haves
- 2–3 years of clinical experience in home health, hospice, or a related field
- Previous leadership or supervisory experience
- OASIS is a plus
- Bachelor of Science in Nursing (BSN) is a plus
- Experience with Kinnser is a plus
