Your days will be filled with purpose as you support and mentor a dedicated team of nurses and caregivers, ensure our patients receive safe, high‑quality, personalized care, oversee clinical documentation, care plans, and regulatory compliance, collaborate with physicians, referral partners, and families, lead case conferences and guide clinical decision‑making, help onboard, train, and inspire new team members, and keep a close eye on quality measures, patient satisfaction, and outcomes. Requirements

  • Active RN license in Michigan
  • Home Health experience required
  • Solid understanding of OASIS and home health regulations
  • Strong clinical judgment and organizational skills
  • Excellent communication and team‑building abilities
  • Ability to multitask and manage competing priorities
  • A passion for supporting nurses and delivering exceptional patient care
  • Ability to work in the field as needed

Nice To Haves

  • OASIS experience strongly preferred
  • Leadership or supervisory experience is a plus

Responsibilities

  • Provide clinical oversight for Home Health nursing staff
  • Ensuring compliance with state, federal, and accreditation standards
  • Reviewing and approving OASIS assessments, care plans, and documentation
  • Supporting field clinicians with education, coaching, and problem‑solving
  • Participating in case conferences and interdisciplinary team meetings
  • Monitoring quality metrics and assisting with performance improvement initiatives
  • Triaging clinical concerns and collaborating with physicians and referral partners
  • Ensuring timely, accurate, and complete documentation
  • Supporting onboarding and ongoing training for clinical staff
  • Promoting a positive, supportive, patient‑centered culture
