About The Position
As a therapist at CenterWell Home Health, you will help patients regain strength, mobility and independence—all from the comfort of their homes. By delivering personalized care that focuses on rehabilitation and functional improvement, you'll empower individuals to overcome limitations, perform everyday activities with confidence and enjoy a better quality of life. You will report to the Clinical Manager. This is a PRN opportunity - schedule/availability to be discussed during interview.
Requirements
- Current and unrestricted Speech Language Pathologist license in the state of North Carolina
- A valid driver's license, auto insurance, and reliable transportation
- Current CPR certification

Responsibilities
- Use professional skills and judgment in evaluating and treating disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize the effects of such disorders and conditions
- Administer and interpret diagnostic tests and applications of therapeutic treatments including audio logic screening
- Submit evaluation, treatment plans, progress reports and discharge summary to the supervisor and care management staff
- Participate in care coordination and discharge planning activities and act as a resource to other health care personnel in meeting patient's needs
- Provide instruction and training to patients in use of alternative communication systems when appropriate
- Provide counsel and instruction to patients, families and healthcare staff
