Elara Caring is seeking a compassionate and skilled Registered Nurse to join our growing Home Health team on a PRN basis. Requirements
- Current, unrestricted Massachusetts Registered Nurse (RN) license
- Minimum of one (1) year of nursing experience in a clinical care setting
- Home Care experience is HIGHLY preferred, as this is PRN and having knowledge of how home care operates will help to make you successful
- Ability to commit to at least one full day (8 hours) per week
- Valid driver's license, reliable transportation, and current auto insurance
- Willingness to travel throughout the assigned territory
- Physical Requirements - Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
Responsibilities
- Completing skilled nursing assessments and developing individualized plans of care
- Performing admissions, routine visits, recertifications, and discharge visits
- Evaluating patients' physical, psychosocial, environmental, and educational needs
- Coordinating care with physicians, caregivers, and interdisciplinary team members
- Monitoring patient progress and identifying changes in condition
- Updating and revising care plans to ensure optimal outcomes
- Educating patients and families on disease management, medications, and self-care techniques
- Maintaining accurate and timely clinical documentation
- Helping prevent avoidable hospitalizations through proactive intervention and patient education
- Delivering compassionate, patient-centered care while promoting safety and independence
