Job Description: Home Health PRN Registered Nurse (RN) Are you an experienced Home Health RN looking for flexibility, autonomy, and the opportunity to make a meaningful impact in your community? Elara Caring is seeking compassionate and skilled Registered Nurses to join our growing Home Health team on a PRN basis. Requirements
- Current, unrestricted Massachusetts Registered Nurse (RN) license
- Minimum of one (1) year of nursing experience in a clinical care setting
- Ability to commit to at least one full day (8 hours) per week
- Valid driver's license, reliable transportation, and current auto insurance
- Willingness to travel throughout the assigned territory
- Associate or Bachelor's Degree in Nursing
Nice To Haves
- Previous Home Health experience strongly preferred
- Hospice experience preferred
- Experience with HomeCare HomeBase (HCHB) preferred
Responsibilities
- Completing skilled nursing assessments and developing individualized plans of care
- Performing admissions, routine visits, recertifications, and discharge visits
- Evaluating patients' physical, psychosocial, environmental, and educational needs
- Coordinating care with physicians, caregivers, and interdisciplinary team members
- Monitoring patient progress and identifying changes in condition
- Updating and revising care plans to ensure optimal outcomes
- Educating patients and families on disease management, medications, and self-care techniques
- Maintaining accurate and timely clinical documentation
- Helping prevent avoidable hospitalizations through proactive intervention and patient education
- Delivering compassionate, patient-centered care while promoting safety and independence
