Job Description: Home Health PRN Registered Nurse (RN) Are you an experienced Home Health RN looking for flexibility, autonomy, and the opportunity to make a meaningful impact in your community? Requirements

  • Current, unrestricted Massachusetts Registered Nurse (RN) license
  • Minimum of one (1) year of nursing experience in a clinical care setting
  • Ability to commit to at least one full day (8 hours) per week
  • Valid driver's license, reliable transportation, and current auto insurance
  • Willingness to travel throughout the assigned territory
  • Associate or Bachelor's Degree in Nursing

Nice To Haves

  • Previous Home Health experience strongly preferred
  • Hospice experience preferred
  • Experience with HomeCare HomeBase (HCHB) preferred

Responsibilities

  • Completing skilled nursing assessments and developing individualized plans of care
  • Performing admissions, routine visits, recertifications, and discharge visits
  • Evaluating patients' physical, psychosocial, environmental, and educational needs
  • Coordinating care with physicians, caregivers, and interdisciplinary team members
  • Monitoring patient progress and identifying changes in condition
  • Updating and revising care plans to ensure optimal outcomes
  • Educating patients and families on disease management, medications, and self-care techniques
  • Maintaining accurate and timely clinical documentation
  • Helping prevent avoidable hospitalizations through proactive intervention and patient education
  • Delivering compassionate, patient-centered care while promoting safety and independence
