The Home Health PM RN Case Manager is responsible for managing caseloads of patients in specialty areas. This role involves conducting initial assessments to gather comprehensive physical, social, and environmental data, and establishing a plan of care in collaboration with physician orders and patient needs. The Case Manager provides direct patient care, adjusts nursing care processes as needed, and identifies problems to modify and evaluate nursing care plans. They also advise and consult with families and other personnel, provide ongoing assessment of patient response to treatments, and update the plan of care as indicated. Documentation, including visit assessments, OASIS, 485, and verbal orders, must be completed accurately and promptly. The role requires communication with physicians to confirm and update care plans, providing status updates, and ensuring ongoing communication with the healthcare team. Facilitating the discharge planning process, participating in chart audits, and continuous quality improvement activities are also key responsibilities. The Case Manager must maintain accurate clinical and patient records according to agency, state, and federal guidelines and demonstrate knowledge of age-specific care principles.
