About The Position

At Residential Home Health and Hospice (‘Residential’), we’re looking to add to our extraordinary care team. This position supports patients in Justice, IL and surrounding areas and is eligible for a $5,000 first year success bonus to reward the accomplishment of key achievements during your first year!

Requirements

  • Associate degree in physical therapy from an accredited college or technical school required.
  • Must hold, in good standing, a license/registration as a Physical Therapist Assistant, issued by the State in which you work.
  • Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
  • Demonstrated experience with tablets, mobile phones and EMR software.
  • Successful completion of a drug screen prior to employment is part of our background process, which includes medical and recreational marijuana.
  • Legally authorized to work in the United States.

Nice To Haves

  • Minimum one year of experience in an acute care and/or rehabilitative setting preferred.
  • Previous home health experience preferred.

Responsibilities

  • Collaborate to provide management and delivery of patient care plans.
  • Support patients in Justice, IL and surrounding areas.
  • Consistently meet critical deadlines for charting.
  • Self-manage time and schedule.
