About The Position
Franciscan Health Lafayette Central is seeking a Home Health Physical Therapy Assistant (PRN) to join their team. This role involves working under the direction of a Physical Therapist to implement therapy plans, provide direct patient care, and monitor patient progress. The ideal candidate is comfortable following and giving instructions, possesses a keen eye for detail to ensure correct patient performance, and is motivated by watching patients improve function and mobility. Requirements
- Associate's Degree from an Accredited Physical Therapy Assistant Program
- Physical Therapy Assistant (PTA) license from the State Licensing Board
- Cardiopulmonary Resuscitation (CPR) certification from the American Heart Association
- Valid Driver's License
- Valid Auto Insurance
- 1 Year of Physical Therapist Assistant Experience
- Up to 75% travel required

- 1 Year of Home Healthcare Experience
Responsibilities
- Report patient progress and collaborate with the evaluating therapist to implement the therapy plan of care.
- Perform comprehensive physical therapy treatment and related follow-up services, including discharge planning.
- Document and submit patient reports within the established timeframe.
- Collaborate with the patient, other healthcare personnel, or outside agencies regarding coordination of care.
- Exercise clinical judgment following an occupational therapy plan of care.
- Train and educate patients and family members or other support, and document understanding of treatment plan and goals.
