About The Position

As a Home Care Physical Therapy Assistant (PTA) you will provide physical therapy services to the organization’s clients under the guidance and supervision of the Physical Therapist, and in accordance with a written plan of treatment established by the physician and physical therapist. Trinity Health At Home – Muskegon is a trusted member of Trinity Health At Home, a national home care, hospice and palliative care organization serving communities across 11 states. As a faith-based, not‑for‑profit agency, we are committed to caring for patients and families in the place they feel most comfortable—at home. Our services include skilled nursing, physical, occupational and speech therapy and medical social work.

Requirements

• Licensed PTA within the State of Michigan
• Graduation from a two-year college-level program approved by the American Physical Therapy Association.
• Minimum of six months experience under a qualified Physical Therapist.
• Must have current Driver’s license and reliable transportation to and from work site.

Nice To Haves

• Home health experience preferred

Responsibilities

• Provide physical therapy services to the organization’s clients under the guidance and supervision of the Physical Therapist, and in accordance with a written plan of treatment established by the physician and physical therapist.
• Function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with the agency’s clients and their families.
