About The Position
Optimal Care is seeking talented individuals for future opportunities as a Home Health Physical Therapist. This role involves providing one-on-one rehabilitation care in patients' homes to help them regain strength, restore mobility, and return to daily activities. The Physical Therapist will assess patients' functional abilities and home environments, develop individualized treatment plans, and guide them through interventions to improve mobility, balance, strength, and safety. The role also includes educating patients and caregivers and working closely with an interdisciplinary care team to help patients achieve their goals and improve their quality of life. This is a high travel position requiring daily travel within the service area.
Requirements
- Master's or doctoral degree from an accredited Physical Therapy program
- Current state license as a Physical Therapist
- Minimum 1 year of experience as a licensed Physical Therapist
- Reliable transportation, valid driver’s license, and valid automobile insurance coverage
- Willingness to travel daily for home visits within service area
Nice To Haves
- Home health experience
- OASIS certification or willingness to obtain
- Experience with Medicare home health documentation
Responsibilities
- Provide one-on-one rehabilitation care in patients' homes.
- Help patients regain strength, restore mobility, and return to daily activities.
- Assess each patient’s functional abilities and home environment.
- Develop individualized treatment plans.
- Guide patients through targeted interventions that improve mobility, balance, strength, and safety.
- Educate and empower patients and caregivers.
- Make measurable progress toward patient goals and a higher quality of life.
- Travel daily within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes.
