About The Position

The Home Health Physical Therapist provides patient-centered therapy in a home health setting in the Denton, Texas area. The role uses the Roger app, a smart, voice-enabled, HIPAA-compliant documentation platform, on most visits to streamline documentation while meeting OASIS and compliance requirements. The position emphasizes compassionate care, patient independence, flexible scheduling, and collaboration with an interdisciplinary team.

Requirements

Active Physical Therapist license in the state of Texas.
Professional liability insurance coverage specific to the Texas Physical Therapy license.
Graduate degree or higher in Physical Therapy from an accredited program.
Strong ability to adapt care approaches to a variety of patient personalities and needs.
Up-to-date knowledge of evidence-based treatment techniques and best practices.
Excellent communication skills and a friendly, professional demeanor when educating and interacting with patients.

Experience in home health or rehabilitative settings is preferred.

Responsibilities

Evaluate patients’ physical abilities and functional limitations in a home health setting.
Develop and implement individualized treatment plans to restore function, improve mobility, and alleviate pain.
Provide therapeutic interventions, including exercise, gait training, manual therapy, and balance retraining.
Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies.
Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes.
Document evaluations, treatment sessions, and progress notes using digital platforms.
Collaborate with interdisciplinary team members, including physicians, nurses, and occupational therapists.
Conduct home safety assessments and recommend ways to prevent falls or re-injury.
Deliver compassionate care while empowering patients to achieve greater independence.
