Requirements
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist (PT) license in the state of practice
- Current CPR Certification
- Valid driver's license and reliable transportation with current auto insurance
Nice To Haves
- Minimum one year of Home Health experience preferred
- HomeCare HomeBase experience preferred
Responsibilities
- Perform comprehensive physical therapy evaluations and establish individualized plans of care.
- Develop and implement treatment programs to improve strength, balance, mobility, and functional independence.
- Reassess patient progress and modify treatment plans to achieve the best outcomes.
- Educate patients and caregivers on therapeutic exercises, safety techniques, and injury prevention.
- Collaborate with physicians, nurses, and interdisciplinary team members to coordinate patient care.
- Maintain accurate, timely documentation in accordance with agency policies and regulatory standards.
- Follow infection prevention and patient safety protocols during every visit.
- Promote exceptional patient care while representing Choice Health at Home with professionalism and compassion.
