Home Health Physical Therapist, Part Time $5,000 Bonus
About The Position
At Mary Free Bed at Home, we’re looking to add to our extraordinary care team. Grounded by our belief that outstanding care is best delivered in a team-based environment, our home health physical therapists collaborate to provide management and delivery of patient care plans. In partnership with Mary Free Bed Rehabilitation Hospital, Mary Free Bed at Home is an accredited recognized leader in home health. We deliver industry-leading therapy and nursing expertise and services that exceed national benchmarks throughout the West Michigan service area. This position supports patients in Holland and surrounding areas and is eligible for a $5,000 first year success bonus to reward the accomplishment of key achievements during your first year!
Requirements
- Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required.
- Licensed Physical Therapist issued by the state in which you will be working.
- Minimum of one year healthcare experience.
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
- Demonstrated experience with tablets, mobile phones and EMR software.
Responsibilities
- Provide management and delivery of patient care plans.
- Collaborate with the care team.
- Consistently meet critical deadlines for charting.
- Self-manage time and schedule.
