About The Position
We are seeking an experienced Home Health Physical Therapist for a 13-week travel assignment serving Seaside, Oregon and surrounding communities. This role is ideal for a therapist who is comfortable working independently in a home health setting while delivering high-quality patient-centered care. This is a travel contract assignment with a day shift, 5x8 hours schedule, from 8:00 AM to 4:30 PM. The assignment is for 13 weeks and is located in Seaside, Oregon and surrounding areas. The setting is Home Health with 1:1 patient care, averaging up to 6 patient visits per day. The productivity expectation is 30 points standard, with a system designed to minimize travel time between patient visits.
Requirements
- Minimum 1 year of Skilled Home Health experience required
- First-time travelers accepted
- BCLS certification required
- OASIS experience required
- Pending license accepted
- Must have reliable personal transportation
Responsibilities
- Evaluate and treat patients in their homes according to physician care plans
- Develop individualized treatment plans focused on mobility, strength, balance, and functional independence
- Complete OASIS documentation accurately and timely
- Educate patients and caregivers on therapy goals, exercises, and safety techniques
- Coordinate care with interdisciplinary healthcare teams
- Maintain compliance with home health regulations and documentation standards
