Requirements
- Bachelor's Degree- Physical Therapy - Required
- 1 Year Physical Medicine/Rehabilitation Experience- Required
- Physical Therapist (PT) State Licensing Board - Required
- Cardiopulmonary Resuscitation (CPR)- American Heart Association - Required
- Driver's License - Required
- Auto Insurance - Required
Nice To Haves
- 1 Year Home Health Experience - Preferred
Responsibilities
- Complete appropriate goal re-assessment and discharge planning.
- Perform comprehensive physical therapy evaluations, treatment, and related follow-up services, including discharge planning.
- Document patient assessments and pertinent information in a timely manner.
- Provide services and treatments requiring physical therapy skills, according to the written plan of care and the signed plan of treatment.
- Supervise Physical Therapist Assistants and Home Health Aides.
- Train and educate patients and family members or others support, and document understanding of treatment plan and goals.
