About The Position

The position provides physical therapy to patients in their homes, using modern tools to support patient independence, flexible workflows, and local impact. Therapists can use the Roger app, a voice-enabled, HIPAA-compliant documentation platform, on most visits. The app is intended to reduce documentation time while supporting OASIS and compliance requirements. The Physical Therapist evaluates patients, develops and implements individualized treatment plans, delivers therapeutic interventions, educates patients and caregivers, monitors progress, and documents care using digital platforms. The role also includes home safety assessments and collaboration with an interdisciplinary care team. Requirements
- Active Physical Therapist license in the state of Texas.
- Professional liability insurance coverage specific to the Texas Physical Therapy license.
- Graduate degree or higher in Physical Therapy from an accredited program.
- Strong ability to adapt care approaches to different patient personalities and needs.
- Up-to-date knowledge of evidence-based treatment techniques and best practices.
- Excellent communication skills and a friendly, professional demeanor when educating and interacting with patients.

Responsibilities
- Evaluate patients’ physical abilities and functional limitations in a home health setting.
- Develop and implement individualized treatment plans to restore function, improve mobility, and alleviate pain.
- Provide therapeutic interventions, including exercise, gait training, manual therapy, and balance retraining.
- Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies.
- Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
- Document evaluations, treatment sessions, and progress notes using digital platforms.
- Collaborate with interdisciplinary team members, including physicians, nurses, and occupational therapists.
- Conduct home safety assessments and recommend ways to prevent falls or re-injury.
- Deliver compassionate care while empowering patients to achieve greater independence.
