About The Position

This position involves providing comprehensive physical therapy services to patients in their homes within North County. The role requires assessing patient needs, developing and implementing treatment plans, and collaborating with patients, families, and other healthcare professionals to achieve optimal patient outcomes. The Physical Therapist will also be responsible for evaluating the home environment, training patients and caregivers, and documenting progress.

Requirements

  • CA Physical Therapy License
  • Two (2)+ Years of physical therapy experience
  • Demonstrate good verbal and written communication and organization skills
  • Possess and maintain current CPR certification
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order

Responsibilities

  • Assess and evaluate therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care
  • Evaluate home environment
  • Assess for muscle strength, mobility, gait, ROM—potential for rehab
  • Direct physical therapy treatment
  • Instruct patients and families/caregivers in the use and care of therapeutic appliances
  • Determine priority needs for physical therapy
  • Report to physician patient’s reaction to treatment or changes in condition
  • Initiate physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient
  • Instruct them as to the goals of the physical therapy program for the patient by participating in case conferences
  • Train patient in the use of prosthetic device
  • Identify patient and family/caregiver needs for other home health services
  • Prepare and submit clinical and progress summaries based on the attainment of goals
  • Participate in discharge planning for patient
  • Prepare and submit a clinical progress summary based on the attainment of goals
  • Provide physical therapy consultation to home families/caregivers
  • Provide in-service education programs for nursing organization personnel
  • Participate in peer consultation process
  • Supervise Physical Therapy Assistants according to organization policy and state regulations
