Requirements

  • Current and unrestricted California Physical Therapy license.
  • CPR/BLS certification.
  • Familiarity with state healthcare regulations and compliance standards.
  • Expertise in modern physical therapy techniques and programs.
  • Strong interpersonal skills to provide compassionate care for diverse patients.
  • Proficiency in treatment assessment and plan development.
  • At least 1 year of recent experience in institutional and home health care settings.
  • Valid driver’s license with an insured, reliable vehicle.
  • Basic computer skills and experience with Electronic Medical Record software (Kinnser preferred).

Responsibilities

  • Plan and deliver skilled physical therapy treatments for patients with injuries, illnesses, or functional disabilities, aiming to maximize their physical potential.
  • Evaluate patients' physical strengths and collaborate with physicians to develop personalized treatment plans.
  • Create and adjust conditioning programs aligned with physicians' treatment plans and patient goals.
  • Communicate with physicians and clinical team members to refine therapy programs and plans of care.
  • Provide clear care instructions for Physical Therapy Assistants and Home Health Aides, and monitor their documentation and performance.
  • Maintain accurate and timely records of patient care, including evaluations, progress reports, and discharge summaries.
  • Report patients' progress to physicians, care managers, staff, and families.
  • Perform other responsibilities as directed by physicians or clinical managers.
