About The Position

Advanced Nursing Concepts provides one-on-one home health care to patients in their homes. We support our clinicians with field mentorship, clinical guidance, and flexible scheduling, so clinicians get the independence of home health without working in isolation. This role involves assessing patients' functional status, carrying out physical therapy plans, instructing patients and families, documenting care, coordinating with physicians, identifying needs for other home health services, participating in discharge planning, providing consultation and education, supervising physical therapy assistants, and checking on home health aide care. The position is full-time and reports to the Branch Director.

Requirements

  • Possesses a degree from a baccalaureate or Master's program in physical therapy approved by an accredited organization.
  • Licensed to practice as physical therapist within the state, if applicable.
  • Two years of appropriate experience as a physical therapist.
  • Demonstrates good verbal and written communication and organization skills.
  • Possesses and maintains current CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
  • Pre-employment physical screening, or a letter from a recent healthcare encounter confirming the candidate is free of communicable disease
  • Documented negative TB test (TST or TB blood test) taken within the past 12 months
  • TB risk assessment and symptom evaluation upon hire

Nice To Haves

  • Community/home health experience is preferred.

Responsibilities

  • Assess each patient's functional status, muscle strength, mobility, gait, ROM, and rehab potential, including the home environment
  • Carry out the physical therapy plan of care set by the physician
  • Instruct patients and families on therapeutic appliances and prosthetic devices
  • Document care accurately and on time, including clinical and progress summaries
  • Coordinate with physicians and report patient response to treatment or changes in condition
  • Identify needs for other home health services and refer as appropriate
  • Participate in discharge planning and case conferences
  • Provide consultation to families/caregivers and inservice education to organization personnel
  • Supervise physical therapy assistants per organization policy and state regulations
  • Check in on home health aide care every two weeks when therapy is the only skilled service
  • Take on other duties as assigned
