About The Position

Choice Health at Home is seeking a Full-Time Home Health Physical Therapist (PT) to join their growing therapy team. The role involves providing personalized rehabilitative care in patients’ homes, helping them improve mobility, regain strength, reduce pain, and achieve greater independence. The therapist will have more independence, meaningful one-on-one patient care, and the opportunity to see the real impact of their work.

Requirements

Graduate of an accredited Physical Therapy program.
Current and valid Physical Therapist license in the state of practice.
Successful completion of applicable state licensure requirements.
Current CPR certification.
Valid driver's license.
Current automobile insurance.

Nice To Haves

One year of home health experience preferred.
Homecare Homebase (HCHB) experience preferred.

Responsibilities

Perform comprehensive initial evaluations to identify functional limitations and establish appropriate therapy goals.
Develop and implement individualized plans of care designed to improve mobility, strength, balance, endurance, and overall function.
Provide therapeutic interventions to relieve pain and prevent or minimize physical disability.
Evaluate patient progress and adjust treatment plans to achieve the best possible outcomes.
Educate patients, families, and caregivers on exercises, mobility techniques, fall prevention, safety, and continued rehabilitation.
Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team.
Apply infection control and universal precaution standards during patient care.
Ensure continuity and quality of care through accurate and timely documentation.
Maintain all clinical documentation within company guidelines.
Promote the agency's philosophy, standards, and commitment to exceptional patient care.
