About The Position

Billet Health is seeking a compassionate Home Health Physical Therapist. Our dedicated, hardworking staff services all of Arizona, parts of Nevada, California, New Mexico and Texas! Our team works with our patients to establish a trusted relationship for care continuum. The right candidate will be able to administer physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager.

Requirements

Graduate from a physical therapist curriculum approved by the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.
Active LPN License in the state of practice
Valid Driver License
Valid Auto Insurance
CPR or BLS Certification
Valid TB Test (Skin test within the last year or x-ray within the last five years)

Responsibilities

Administer physical therapy to patients on an intermittent basis in their place of residence.
Perform duties in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager.
